This travel PT role provides specialized physical therapy services within home health and hospice settings as part of an interdisciplinary team. The incumbent will deliver patient-centered care, ensuring safety, regulatory compliance, and measurable progress toward rehabilitation goals for geriatric populations. While serving in a travel capacity, the clinician will work across patient homes, office settings, and community environments as appropriate.

Responsibilities
- Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans
- Provide therapeutic interventions to improve mobility, strength, pain control, and functional independence
- Supervise and coordinate care with Physical Therapist Assistants as applicable
- Collaborate with an interdisciplinary team to coordinate patient-centered care and discharge planning
- Educate patients and caregivers on safe mobility techniques, equipment use, and home exercise programs
- Document care, monitor outcomes, and adjust treatment plans under medical direction and organizational policies
Requirements
- Active Physical Therapist licensure in North Dakota
- Current driver’s license and access to a vehicle to travel to patient homes
- 2 years of prior PT experience strongly preferred
- Ability to work in home health, outpatient, and office/community settings as assigned
- Willingness to travel within assignment requirements and to coordinate care with the care team
