Wraparound Care Coordinator The Wraparound program works with youth aged 12-20 with a diagnosed mental health disorder and/or developmental delay who are approved for this service through the ‘Single Assessment’ process. Wraparound provides comprehensive, holistic, youth and family‑driven intensive case management services through the development of a team comprised of both professionals and natural supports. Job Overview The Wraparound Care Coordinator is a direct service staff position in the community services area. The role contributes to the agency’s mission by providing home‑based and community‑based case management services to youth and families with a mental health diagnosis or developmental disability. Responsibilities Provide home‑based and community‑based case management services to youth and families. Attend monthly staff meetings. Participate in weekly supervision with the designated supervisor. Maintain accurate case documentation and paperwork, both on‑site and remotely. Coordinate transportation and travel to meetings, trainings, and client visits, including driving when necessary; a good driving record is required. Work flexible hours, including evenings, as required. Schedule Full‑time, 40 hours per week. Monday–Friday with some evening hours may be required. Location Office located in South Portland. Hybrid schedule – some in‑office and some remote work. Qualifications Bachelor’s Degree in Social Work or a related field (required). At least two years’ experience working with families and children, preferably within the mental health system. Knowledge of serious and persistent mental illness, substance abuse, and trauma. Ability to work independently and collaboratively. Ability to work flexible hours, including evenings. Must meet physical requirements associated with family visitation (e.g., climb stairs); able to exert moderate physical effort such as standing, walking, reaching, stooping, bending, kneeling, crouching, typing, and frequently lift and carry small children or items up to 25 pounds. Computer proficiency with Microsoft Office and email. Must successfully pass a criminal background check, child protective service check, and sex offender check. Must not be on the state or federal suspension and disbarment list. Driving may be necessary to attend meetings, trainings, and client visits; a good driving record is required.
