Overview Bilingual Case Management Coordinator (Field-Based) Salary: $36.00 / hour Location: Remote / Work from Home (Must reside in Miami, FL ) Travel: 75% regional travel required (Home visits, ALFs, and nursing homes) Schedule: Monday – Friday (Enjoy autonomy and flexibility!) Responsibilities Comprehensive Assessment: Conduct telephonic and face-to-face evaluations of member medical needs in their homes or care facilities. Care Coordination: Develop and implement action plans to enhance member outcomes through service integration. Clinical Support: Manage prior authorizations, coordinate with PCPs and skilled providers, and perform medication reviews. Resource Advocacy: Connect members with essential community resources and condition management information. Documentation: Maintain highly organized records within a fast-paced, changing environment. Qualifications Bilingual Fluency: Professional proficiency in both Spanish and English is REQUIRED. Residency: Must currently reside in the area. Mobility: Ability to travel approximately 75% of the time within the region to see members in person. Tech-Savvy: Comfortable with remote training (Microsoft Teams) and managing digital caseloads. Preferred Qualifications Exceptional multitasking and prioritization skills. Strong verbal and written communication. A detail-oriented mindset with the ability to work autonomously.
